Immunization Compliance Office

The Immunization Compliance Office is here to assist students with the registration process. Our undergraduate timeline of admissions will help guide you step by step through the process.

USF requires students to submit the Immunization Health History Form and present immunization documentation for the following:

·         (2) Measles 

·         Rubella

·         Hepatitis B or waiver

·         Meningitis or waiver if student is living off campus

The office is located in the SHS Annex and is open throughout the year, Monday through Friday from 8:00am to 5:00pm.
