The IM and MH hold stands for the immunization requirement and the USF Immunization Health History Form. To remove these holds, refer to the immunization requirements.
What immunizations are required for students living on campus?
Measles and Rubella immunizations are required for EVERYONE born after December 31, 1956. Any combination of two doses of Measles and one dose of Rubella will satisfy this requirement. Refer to Section A of the Instructions for Immunization Health History Form for specific qualifications.
Students living in USF Housing MUST be vaccinated against Meningitis. No room will be assigned without proof of vaccination. ALL STUDENTS are required to read the following information about Hepatitis B and make a choice to receive the vaccine or decline receipt of the vaccine.

What immunizations are required for students living off campus?

Do online students have to comply with the immunization requirements?
If ALL of your courses are online and do not meet on campus for any reason (orientation, tests, etc), then you do not have to comply with the immunization requirements. The hold will remain on your OASIS account, but will not prevent you from registering for online classes.
Where do I find the necessary immunization records?
USF will accept the official State of Florida Immunization Form (Form 680) issued by local health departments and physicians’ offices, in conjunction with completing the personal information and signing the Immunization Health History form. High school transcripts, state university records, or childhood shot records are also acceptable as proof.

What are my options if I cannot find my records?
We offer the shots and the blood test (IgG titers) on campus at Student Health Services Annex (Immunization Compliance Office). The shots are offered on a walk-in basis Monday through Friday 8:00 am until 4:30 pm. The MMR immunization is $55 per shot and the titer test is $70. Some insurance is accepted based on their coverage.
You can also go to your own physician’s office, walk-in clinic, or the health department to fulfill the immunization requirements.
How long does it take for a hold to be lifted?
If the USF Immunization Health History form and the immunization records are in compliance, the hold will be removed the same day if received by 4:00 pm. If received after 4:00 pm, the hold will be lifted the following business day.
If you have just received the first dose of MMR, you will get an override for exactly one month from that date.
Does the USF Immunization Health History form require an official physician’s stamp and signature?
YES. An MD office, clinic, or health department “official stamp” AND an authorized signature must be included for this document to be complete and approved. Alternate acceptable records which may be attached to this form include health department records, official State of Florida Immunization Form (Form 680), doctor’s records or school records.
Where can I obtain a copy of my previously submitted immunization records?
You can print an official copy of the records that were required from your OASIS account under the Personal Information tab. You can also call (813)974-4056 to have the records faxed or mailed.
When are the Immunization requirements due?
You can avoid registration cancellation by submitting the requirements by the fifth day of classes. However, if you cannot register you will need to submit the requirements before you are able to register.
