Student Health Services uses the USF Immunization Health History Form for evaluating and maintaining medical requirements for registration for all University of South Florida students. If you still have questions about this form, call the Immunization Compliance Office at 813-974-4056 or E-mail to immunization@shs.usf.edu. Carefully read and follow the instructions then complete, print, and return the form to:
